Abstract

Original in germ line cells, ovarian teratoma is frequent in childhood, young and adult women before the age of 40. Mature teratoma, also known as a dermoid cyst, is among the most common ovarian masses, with an incidence of up to 58% of benign ovarian tumors. Frequently diagnosed during the routine gynecological examination, the symptomatology of those ovarian tumors is mild and nonspecific. Proper evaluation of a suspected teratoma should be performed to establish the benign or malignant characteristics of the tumor. Though with a low risk of recurrence, especially within the first year after the first encounter, the reccurrence rate is up to 21% and can occur even after 15 years. Surgical treatment is the gold standard in the management of this medical condition. Patients should be routinely evaluated for recurrences and possible complications.

We present a case of a 35 years old woman with 10 years apart contralateral recurrence of a dermoid cyst. Before the second encounter, the patient had regular examinations with no abnormal findings. This case is particular because of the prolonged timeframe to the second diagnosis. The initial characteristics of the dermoid cyst were considered to be at high risk for early recurrence within the first year following the first surgical excision.
